The Complexity of Back Pain


The research tells us that there is not one exercise that helps those with back pain more than any other. No core exercise is better than a deadlift, Pilates is no better than yoga, and Hyrox is not better than regular cardiovascular exercise.


It shows that time heals the majority of non-specific lower back pain. Yet, pain clinics are full of individuals who have not found relief. Why is that? Often, there are other reasons behind long-term pain.


So, what's the truth?


With over 10 years of treating the general public, I've learned that the biggest frustrations arise when people feel unheard. When their treatment plan isn't tailored to them, it can lead to feelings of dismissal. Hearing phrases like "back pain gets better no matter what you do" or "just keep moving" can be disheartening. While there’s some truth to these statements, they don't address the unique challenges each person faces.


The Importance of Listening


In my experience, when a person comes to me with specific problems, they deserve a specific solution. I spent two years working in a community pain clinic, and I was astounded by how many patients simply needed their feelings validated. They wanted a clear plan to return to their daily activities.


Sometimes, the solutions were straightforward. For instance, if someone said, "I need to be able to twist to reach into a cupboard," we could easily implement standing rotational and flexion exercises with progressive loads. Other times, the issues were more complex, involving significant fear and avoidance. In such cases, Cognitive Functional Therapy (CFT) approaches were necessary.


Avoiding the Research Physio Trap


The summary of my post is this: don’t fall into the research physio trap. Be specific. This doesn’t mean we need to dive deep into biomechanics, which can often confuse patients and may not be clinically relevant in causing pain. Instead, we must listen to what the patient is reporting and offer a tailored plan aimed at improving their specific issues.


The research is there as a guide. It’s not an absolute. Much of the research available has its flaws. What seems clear is that time is a fantastic healer. Back pain responds well to exercise, ibuprofen, and poorly to avoidance. That’s the crux of it.


What If Time Doesn’t Heal?


You may encounter individuals who don’t improve with time, aren’t exercising, and can’t use ibuprofen. Where does that leave you and the patient? The advice to "keep moving" simply won’t suffice.


So, why should you consider physiotherapy? The right physiotherapist will help you prevent strength losses and functional declines. They will guide you on timeframes, help exclude red flag pathologies (see other blog post), and educate you. This empowerment can significantly mitigate the effects of pain on your life. It can be worth its weight in gold when done correctly.
