Acute Back Pain: What to Do in the First 7 Days (and What to Avoid)

Most acute back pain episodes improve significantly over days to weeks. That doesn’t mean it’s ‘nothing’ — it often means the spine is irritated and protective, not permanently damaged.

What’s happening during a flare

  • Local tissue irritation (joints/disc/ligaments)

  • Muscle guarding and stiffness

  • A sensitised nervous system (pain alarm turned up)

  • Reduced confidence in movement (which can increase protection)

What to avoid (common mistakes)

  • Complete bed rest for days

  • Repeatedly ‘testing’ the same painful movement

  • Aggressive stretching into sharp pain early on

  • Trying to hold a perfect posture all day — aim for variety

A simple first 7‑day plan

Days 1–2: calm it down

  • Move little and often (short walks, gentle position changes)

  • Use heat if it helps

  • Pick 1–2 gentle movements that reduce symptoms

Days 3–5: restore normal movement

  • Gradually increase walking time

  • Reintroduce daily tasks with pacing

  • Start light strengthening if tolerated

Days 6–7: build confidence and capacity

  • Increase activity in small steps

  • Begin a progressive plan (strength + mobility + return to work/sport tasks)

Red flags — seek urgent medical help now if you have

  • Bladder/bowel changes or saddle numbness

  • Progressive weakness

  • Fever/unwell, unexplained weight loss, history of cancer

  • Significant trauma (fall/accident) or suspected fracture risk
